The Early Field Experiences Assessment Form is used to assess student performance in the field site. The form is filled out by the pre-service teacher's mentor and contains the following identifying information:
- Field experience course (either C I 295A or C I 295B)
- Name of pre-service teacher
- Semester of field experience
- School/Center
- Mentor name
- Supervisor
The student is rated based on their performance in six tasks:
- The pre-service teacher arrives at the field experience site punctually.
- The pre-service teacher carries out assigned tasks dependably and willingly.
- The pre-service teacher maintains an appropriate personal appearance.
- The pre-service teacher interacts positively, tactfully and professionally with mentor, colleagues, parents, students, and others.
- The pre-service teacher treats information about students, colleagues and the school confidentially.
- The pre-service teacher seems interested in learning and takes advantage of learning opportunities.
These ratings should be delivered on the following scale:
- Consistently -- The pre-service teacher’s performance always or almost always matches or exceeds the description.
- Often – The pre-service teacher’s performance on this standard matches or exceeds the description most of the time but falls below now and then
- Sometimes -- The pre-service teacher’s performance on this standard matches or exceeds the description about half or a little more than half of the time
- Rarely -- The pre-service teacher’s performance on this standard fails to match or exceed the description more than half of the time
